Risankizumab (Skyrizi)

Overview

Risankizumab is a newer biologic that selectively targets IL-23 (specifically its p19 subunit) without blocking IL-12, providing more targeted suppression of the Th17 inflammatory pathway. It is approved for moderate-to-severe Crohn's disease and ulcerative colitis.

Key Uses

1

Moderate-to-severe Crohn's disease

2

Moderate-to-severe ulcerative colitis

3

Patients who have failed or are intolerant to conventional therapy or biologics

4

Also approved for plaque psoriasis

How It Works

•

Selectively binds the p19 subunit of IL-23 — spares IL-12, preserving some protective immune functions

•

Reduces Th17 cell activity and downstream inflammation in the gut

•

Induction (Crohn's): IV infusions at weeks 0, 4, and 8; maintenance: 360 mg SC every 8 weeks

•

Induction (UC): 1200 mg IV at week 0; maintenance: 180 or 360 mg SC every 8 weeks
