Certolizumab Pegol (Cimzia)

Overview

Certolizumab pegol is a PEGylated Fab' fragment of a humanised anti-TNF-α antibody. Unlike other anti-TNF agents, it lacks an Fc region, which means it does not activate complement or cause antibody-dependent cell-mediated cytotoxicity. It is approved for Crohn's disease and is notably used in pregnancy because minimal drug crosses the placenta.

Key Uses

1

Moderate-to-severe Crohn's disease

2

Particularly suitable for use in pregnancy (minimal placental transfer)

3

Also approved for rheumatoid arthritis, psoriatic arthritis, and axial spondyloarthritis

How It Works

•

Binds and neutralises TNF-α without Fc-mediated effector functions

•

PEGylation extends its half-life, allowing less frequent dosing

•

Induction: 400 mg SC at weeks 0, 2, and 4; maintenance: 400 mg SC every 4 weeks (or 200 mg every 2 weeks)

•

Low placental transfer makes it the preferred anti-TNF in pregnancy when treatment is needed
